During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 7/21/25. The on-site inspection began at 8:30am and ended at 11:30am. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 36 children present and 9 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/4/25. A POC submitted after this date will not appear on the public website.
Violations
7The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a safe and operable condition. The crib for child #4 sat uneven on the floor. A wheel was broken off one of the crib legs. The evacuation crib had a loose bolt causing the crib foot and sides to shake when moved.
Hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children. In the school age classroom there was an unlocked closet with cans of latex paint and pesticide spray. In an unlocked space under the Directors desk there were cleaning products and laundry detergent.
Up-to-date information is required to be requested from the parent for each child's record at least annually. The information in the record of child #1, child #2 and child #3 had not been updated in over 14 months.
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and include all of the required information. The injury records for three children did not contain all of the required information.
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #4, enrolled for ten months, was missing a parent work phone number.
Staff records shall be kept for each staff person and contain all the required information. The staff records for staff #1 and staff #2 did not contain all of the required information.
