Plan of Correction: The facility responded: When enrolling students or auditing files, the Director will check each form for immunizations and physicals have the required signature or stamp included.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a safe condition. There was a staple gun in a plastic drawer, approximately 3 feet high in a preschool room.

Plan of Correction: The facility responded: The staple gun was removed and placed in a locked cabinet. Staff were reminded to always place items back in there designated spaces when not in use.

Toilet chairs shall be emptied promptly and cleaned and sanitized after each use. A toilet chair in a preschool bathroom was not emptied after use. There was dried urine in the potty bowl.
Plan of Correction: The facility responded: Staff were given repeat training on disposing of urine in potty chairs and to ensure after each use the chair is cleaned and sanitized. Openers and closers will incorporate checking the potty chairs in their walk-throughs.

Documentation shall be maintained of shelter-in-place and lockdown drills that includes all of the required information. The drill logs for the shelter in place and lockdown drills conducted in 2025 did not include the number of children and staff that had participated.
