During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 8/6/25. The on-site inspection began at 11:30am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 caregiver; the point total was 9. The inspector reviewed 4 children's records and 2 caregiver/household member records on-site. This inspection included document review, a tour of the home, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. The record for child 1, who has been in care for over 30 days, does not contain documentation of a physical examination.
Plan of Correction: The provider responded with the following: The provider spoke with child 1's parent during the inspection. The parent will provide a copy of the child's most recent physical exam at drop-off tomorrow.
