Inspector Notes
An unannounced monitoring inspection was conducted on 11/29/2023 from1:30pm-2:50pm. At the time of the inspection there were 5 children in care and the licensee. Total points present was 12. Children were observed during nap and eating snack. Emergency supplies, evacuation drills, and records for the caregiver, children and household members were reviewed. The outside play space was also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Based upon review of children's records and provider interview, it was determined the provider did not ensure that each child's record includes the names, addresses and telephone numbers of two persons to be contacted in an emergency when a parent cannot be reached
1.The records for child #1 and #5 did not contain the home address of the emergency contacts.
2.The record for child #2 did not contain the phone number of an emergency contact.
1.The provider could not provide documentation that emergency evacuation, shelter in place or lockdown drills were conducted prior to November 2023.
2.The provider stated she had recently thrown out the drill logs
