Inspector Notes

A renewal inspection was conducted on 9/13/23 from 10:45 am to 1:10 pm. There were 25 children present, with 8 staff supervising. The children were observed during diaper changes, eating lunch and napping. The lunch menu included rice, beans, corn, cole slaw, melon and milk. Handwashing was observed after diapering and before eating lunch. 4 staff records, 3 children's records, medications, emergency drill records, injury reports, attendance and other center documents were reviewed. The outdoor play area was also assessed for compliance. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website. Based on observation, rest mats were not identified for use by a specific child.
Evidence: 5 rest mats in the Two's classroom were not labeled with each child's name.
Plan of Correction: All rest mats will be labeled with children's names.
Standard 8VAC20-780-550-P
Based on records review, written record of children's serious and minor injuries did not contain complete information.
Evidence: A review of four incident reports from July and August 2023 showed that they were missing the following information: time of injury, full name of child, future prevention plan, action taken, time of notification to parents and parent signature.
Plan of Correction: Staff will be retrained on how to complete injury reports. Injury reports will be reviewed to make sure they are accurate and complete.
