Inspector Notes
A complaint inspection was initiated and concluded on 12/14/23. A complaint was received by the department regarding allegations in the areas of ratios and supervision in the Toddler classroom, and the reporting of injuries and illnesses. 5 staff members were interviewed, children's attendance reports and injury reports were reviewed. The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and violations were issued. Violations
1Based on records review and staff interview, the center did not maintain a written record of children's injuries.
Evidence:
1. An injury report dated 12/13/23 for Child # 1 was found in the Toddler classroom.
2. The injury report was missing the following information: Time of injury, staff / parent signatures, nature of injury, preventative methods and steps taken to treat the injury.
3. Staff # 1 stated that the injury occurred at approximately 10:45 am, and she was unable to complete the report.
