Inspector Notes

An unannounced onsite inspection was conducted today. A total of 10 children with the Provider, one assistant and one volunteer was present. When the Inspector arrived, the children were observed engaged in a dancing activity. Handwashing was observed prior to their transition to lunch. Lunch today was chicken, rice and vegetables with water to drink. Staff observed attending to infants, preparing infant bottles and holding one infant while bottle feeding. Observation of the crib room, bathroom, nap room and large programming room was conducted. A total of 2 children's records and staff training was reviewed. No new staff/household members since the last inspection in October 2021 and all background checks remain current. Some violations found today. Thank you for your cooperation today. Standard 8VAC20-800-210-A
Based on review of staff records, annual staff training was not obtained.
Evidence:
1.Staff #1 and #2 received only 3 hours of training since the last inspection in October 2021 when it was previously cited.
2. Staff #1 has received only 4 hours out of the required 16 hours of annual training.
3. Staff #2 has received only 3 hours of the required 16 hours of annual training.

Standard 8VAC20-800-480-D
Based on observation of lunch, equipment used by at least child was not used in accordance with the manufacturer's instructions.
Evidence:
1. Child #3 is over the age of 3 years.
2. Child #3 was observed sitting in a Fisher Price portable high chair at lunch.
3. The manufacturer's instructions for use of this equipment is for children aged 0 to 36 months.
4. Two children were observed standing up while harnessed in the portable high chairs and one of the two children was observed falling over while harnessed in the portable high chair. Posing a potential injury to this child.
Plan of Correction: No children 3 years and older will be using the portable high chairs. Any child who is able to stand up and/or walk will no longer use a portable high chair.

Based on observation of the crib room, one crib did not have a tight fitting sheet.
Evidence:
The middle crib located in the crib room had a sheet that was not sufficiently tight on the mattress.
Plan of Correction: I will replace this sheet today with a tighter fitting sheet.
Standard 8VAC20-800-690-D
Based on inspection of the diapering surface, the surface was not nonabsorbent.
Evidence:
The diaper surface currently in use has two holes in the plastic covering which exposes the foam padding which is no longer nonabsorbent and can not be properly sanitized.
Plan of Correction: I will replace this diaper surface this weekend.

Based on review of the provider's emergency drills, shelter-in-place procedures were not practiced twice a year.
Evidence:
The provider failed to complete 2 shelter-in-place practice drills in 2021.
Plan of Correction: A shelter in place drill will be completed this month and one more before December 2021 and twice a year ongoing.
