An unannounced, on-site monitoring inspection was initiated on 7/28/2025 and completed on 7/28/2025. The on-site inspection began at 10:15AM and ended at 10:45AM. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver; the point total was 8. The inspector reviewed 3 children?s records and caregiver/household member records.

A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for Household Member #1 was not completed and was overdue by nine months.
Plan of Correction: It will be completed right away and sent to the licensing inspector.
