Standard 22.1-289.035-B-3
The center must request a search of the central registry prior to the employee's first day of employment. Staff #2, who had been employed for more than one year, did not have a central registry search requested.
Plan of Correction: The central registry search for staff #2 was requested at the time of hire but never received. Upon notification, the CRS was obtained and placed in the employees file. Moving forward the center will ensure all CRS requests are submitted, received and documented with in the required time frame. The director will be responsible for monitoring compliance.

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of Child #3, who had been enrolled for three months.
Plan of Correction: The physical exam for child #3 was received and placed in child's file. Moving forward, the center will ensure all physical examinations are received prior to child's start date. Child records will be reviewed upon enrollment and monitored by administration to ensure compliance.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. A container of cleaning products with Keep out of the Reach of Children was placed in an unlocked cabinet underneath the hand sink located in Classroom #1. Children were present in the classroom during the inspection.
Plan of Correction: Container of cleaning chemicals immediately moved to locked cabinet. Moving forward, all hazardous materials will be kept in locked cabinets at all times. Classroom Storage areas will be checked daily by staff and monitored by administration to ensure compliance.
