When a group of children receiving care includes children from different age brackets, the age of the youngest child in the group shall be used to determine the staff-to-children ratio that applies to that group. Upon arrival there were 11 children with two staff. The children present ranged in age from 11months through 6 years old. The number and ages range of children present required one additional staff.

Annually, the exempt facility will document that each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. The record for Staff #1 and #2 indicate that the health statements are past due for each staff.

The child day center is required to demonstrate compliance with all safe sleep guidelines recommended by the American Academy of Pediatrics. During the inspection an infant was observed sleeping on top of a soft cushion with a fiber filled sleeping bag stacked on top of cot.

Ensuring that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining sand or other cushioning material under playground equipment. A child under the age of two years old was cared for in rooms without direct egress to outside.
