Based on child record review, the provider failed to obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
The record for C9 (under the age of 2 years) did not contain documentation of immunizations every six months. The immunizations documented in the record were dated 03/14/2022 and 05/24/2023.

Based on staff record review, the provider failed to ensure that two of nine staff submit documentation of a negative tuberculosis screening at the time of employment or documentation completed within the last 30 calendar days of the date of employment.

Evidence:
The record for S3, employed 03/27/2023 did not document a tuberculosis (TB) screening within the required time frame. The screening was completed 01/19/2023.
The record for S6, employed 03/14/2023 did not document a tuberculosis (TB) screening.

Based on record review, the provider failed to ensure that staff shall complete the Virginia
Department of Education-sponsored orientation course within 90 days of employment.
Evidence:
The records for S2, employed 09/28/2020 and S8, employed 03/06/2023 did not contain a certificate of showing completion of the Virginia Preservice Training.

Based on interview, the provider failed to ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present.

Evidence:
During an observation in the pre-school classroom, the licensing inspector asked the program director to identify the program leader in the classroom. The program director reported that the classroom did not have a program leader due to the teacher leaving employment.

Based on observation the center failed to ensure that children under 10 years of age
always shall be within actual sight and sound supervision of staff.
Evidence:
A child, age 6 was observed by licensing inspector and program director coming out of a
hallway bathroom in the school age building while her teachers and classmates were in a classroom down the hallway with the door closed. The program director intervened and escorted the child to the classroom to be reunited with the class and teachers. The child?s teacher reported to the program director that she counted her in the hallway when they were moving the class but did not realize the child had gone into the bathroom.

Based on observation, the provider failed to ensure food brought from home was clearly
dated and labeled in a way that identifies the owner.
Evidence:
Licensing inspector observed infant bottles for three infants in the Infant I room refrigerator that were not dated.

Based on child record review, the provider failed to ensure that five of 10 records contained required documentation.
