Inspector Notes
A monitoring inspection was conducted on 06/06/2023 from approximately 11:40 a.m. ? 5:20 p.m. There were 142 children present ranging from three months to school age and 22 staff supervising. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Ten child records and nine staff records were reviewed.
The information gathered during the inspection determined violations with applicable standards or law. A violation notice was issued. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 07/3/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). The record for C9 (under the age of 2 years) did not contain documentation of immunizations every six months. The immunizations documented in the record were dated 03/14/2022 and 05/24/2023.

During an observation in the pre-school classroom, the licensing inspector asked the program director to identify the program leader in the classroom. The program director reported that the classroom did not have a program leader due to the teacher leaving employment.

A child, age 6 was observed by licensing inspector and program director coming out of a
hallway bathroom in the school age building while her teachers and classmates were in a classroom down the hallway with the door closed. The program director intervened and escorted the child to the classroom to be reunited with the class and teachers. The child?s teacher reported to the program director that she counted her in the hallway when they were moving the class but did not realize the child had gone into the bathroom.
