Inspector Notes

An inspection was conducted with an outside agency on 06/14/2024 from 9:30 a.m. to 11:20 a.m. The inspector reviewed compliance in the areas of administration, staff and supervision, programs, and background checks. Three staff were interviewed, video footage and documentation were reviewed, and two staff and one child record were reviewed. The information gathered during the inspection determined a violation with applicable standards or law. Please sign and complete the ?plan of correction? and ?date to be corrected? for the violation cited on the violation notice and return it to me within 5 business days. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Based on interviews, documentation, and review of video footage, the center failed to ensure that behavioral guidance shall be constructive in nature, age & stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

Evidence:
On 06/03/2024, staff S2 was observed on video attempting to wake C1 from a nap. S2 was observed lifting the long side of the cot and rolling C1 off the cot onto the floor. C1 reported that she was sleeping, and it was scary, and that the lady at the school woke her up, and that it hurt
