Inspector Notes

The inspection findings have been updated as a result of a first-step review completed on November 26, 2025.

An unannounced inspection was initiated on 10/1/2025 and completed on 10/01/2025 with a local agency in response to a complaint received by the licensing office on 9/18/2025 relating to allegations of rough handling, behavior guidance, and staffing and supervision.The inspector reviewed three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 11/4/2025 . A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

Staff #1 and Staff #2 did not follow discipline policy based on the center's policies and procedures for appropriate behavior management and redirection.
Plan of Correction: ln both situations cited, the employees were attempting to prevent injury and reacted quickly, with no malicious intent. Positive guidance and redirection procedures have been reviewed with all staff. Management will monitor child/adult interactions daily through classroom observations and offer support and coaching when needed.

Behavioral guidance shall be constructive in nature, age and state appropriate and be
intended to redirect children to appropriate behavior and resolve conflict.

During interviews on 10/01/2025, Staff #1 admitted to grabbing a child on the upper
arm. Staff #1 stated, ?she was grabbing child on the upper arm when he was not on his
cot to move him over to cot.? Staff #2 acknowledged during an interview on 10/01/2025 of pulling a child by the wrist to get him to sit down.?
