8VAC20-780 Staffing and Supervision.

During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 10/27/2025 and completed on 10/27/2025 in response to a complaint received by the licensing office on 10/9/2025 and 10/23/2025 relating to allegations of supervision and ratio. This inspection included document review, tour of the facility, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 11/10/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-350-B-1
The required ratio for children ages birth-16months is one staff per four children.

1. On 10/23/2025, Infant classroom attendance sheets identified one staff member supervising five children from 7-8am. One staff member supervising ten infant children from 8-9am. Three staff members supervising thirteen infants from 9am to an undetermined time. Two staff members supervising ten infants from 3pm to an undetermined time.

2. On 10/27/2025, two staff were observed supervising nine children (youngest child four months old). The required ratio is one staff per four children.
Plan of Correction: Ratio requirements for infant program have been reviewed with all staff and managers. Staff schedules have been adjusted to accommodate attendance/drop off flow of children. If classroom is at ratio capacity, children will not be accepted into care until another staff member is available.

The required ratio for children ages 16 to 24 month is one staff per five children.

1. On 10/06/2025, Two's classroom attendance sheets identified one staff member supervising nine children from 8-9am.

2. On 10/07/2025, Two's classroom attendance sheets identified one staff member supervising eleven children from 8-9am.

3. On 10/23/2025, Two's classroom attendance sheets identified one staff member supervising fourteen children from 10-11am.
Plan of Correction: Ratio requirements for two year old program have been reviewed with all staff and managers. Staff schedules have been adjusted to accommodate attendance/drop off flow of children. 1. On 10/07/2025, Pre-K classroom attendance sheets identified one staff member supervising twelve children from 9-10am. One staff member supervising eleven children from 10-12pm. One staff member supervising eleven children from 3-4pm.

2. On 10/23/2025, Pre-K1 classroom attendance sheets identified one staff member supervising twelve children from 9-10am. Pre-K2 classroom attendance sheets identified two staff members supervising twenty-one children from 9am until approximately 4pm.
