During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site inspection was conducted 11/21/2024 from 1:55pm- 4:30pm. The inspector reviewed compliance in the areas listed above. There were 130 children present and 22 staff members. The inspector reviewed 11 children?s records and 3 staff records onsite. This inspection included document review, interviews, observations, and a tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today. A POC submitted after this date will not appear on the public website.
Violations
2Staff #1 did not undergo a background check every five years. The results of a central registry check were obtained more than five months after expiration. The results of a criminal history check were obtained more than six months after expiration. The sworn statement was updated more than three months after expiration.
The record for Staff #1 contained subsequent TB results dated more than one month after expiration.
