Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the home.

A subsidy health and safety inspection (SHSI) was initiated on February 18, 2022 and concluded on February 25, 2022. There were 2 children present with 1 caregiver( supervising. A monitoring inspection was also conducted on this date.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Based on documentation review, the provider failed to have documentation of immunizations for one child in care.

Evidence:
1. Two children records were reviewed.
2. Child #2 did not have documentation of immunizations.
3. The provider confirmed she did not have documentation of immunizations for Child #2.
Plan of Correction: I will request documentation of immunizations from the parent.
