Inspector Notes
Today an unannounced Monitoring Inspection took place between the hours of 11:15am and 1:20pm. There were 2 classrooms observed with a total of 18 children with 3 staff within the supervision guidelines. The children were observed doing flashcards, washing their hands, eating snack and resting. A complete inspection of the physical plant, children and staff records, fire drill log and emergency procedures were observed during this inspection. Violations were found during todays inspection and can be viewed on the violation notice.

Violations
8Evidence: Staff #1's record contains documentation of 10 hours of training completed in 2021.
Evidence: During the inspection, there was a group of 14 children with 2 staff. The program director confirmed that the youngest child in the group was 22 months old.
Evidence:
1. The children were observed resting at approximately 1:05pm.
2. Staff #1 was left alone with 14 children for approximately 2 minutes when when Staff #2 left the classroom.
3. Staff #2 was left alone with 14 children for approximately 2 minutes when Staff #1 left the classroom.
4. The program director confirmed that the youngest child in the room is 22 months old.
Evidence:
1. The menu was not posted in a location conspicuous to parents; the food menu was stored in a closed cabinet.
2. The menu was not dated.
Evidence: At the start of the inspection, the children were observed sitting at the table coloring and working on flash cards. Approximately an hour later, lunch was served while the children were still seated at the table. The table was not sanitized before the children ate lunch.
Evidence: During lunch, the staff were observed serving the children one item from their meal at a time; beef meatballs and salad, then porridge, then bread, then beverages. The staff did not sit with the children while the children ate.
Evidence:
1. Child #2's record does not contain documentation of addresses for both emergency contacts.
2. Child #1, Child #2 and Child #3's records do not contain documentation of each child's first date of enrollment.
3. Child #3 (date of enrollment: 8/12/2020) and Child #4's (date of enrollment: 9/1/2020) records do not contain documentation of an annual review of parental and emergency contact information.
Evidence:
1. Staff #1, Staff #2 and Staff #3's records do not contain documentation to demonstrate that the individuals possess the education, certification or experience required by the job positions.
2. Staff #3's record does not contain documentation that two or more references as to the character and reputation as well as the competency were checked prior to employment.
