During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on October 7, 2025, as a part of the provisional licensure period. The on-site inspection began at 12:00 p.m. and ended at 1:05 p.m. The inspector reviewed compliance in the areas listed above. There were 17 children present and 4 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 27, 2025. A POC submitted after this date will not appear on the public website.

Orientation training shall include prevention of sudden infant death syndrome and use of safe sleep practice, prevention of shaken baby syndrome and abusive head trauma, including procedures to cope with crying babies or distraught children, prevention of and response to emergencies due to food and other allergic reactions, and prevention and control of disease.

Areas and equipment of the center, inside, shall be maintained in a safe and operable condition.

There were multiple areas of chipped paint located on a wall in the 2 to 5 year old classroom within reach of children. The gate leading to the kitchen was inoperable. There were three knives in an unlocked drawer in the kitchen, accessible to children.

The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies. This list shall be dated and kept confidential in each room or area where children are present.

Child #2's (age: 4 years) record indicates that the child has twelve food allergies. Child #2 was not listed on the center's allergy list. The allergy list posted on the refrigerator was not dated or kept confidential. There was no allergy list in the second classroom.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: name, address and phone number of two designated people to call in an emergency if a parent cannot be reached.

Child #4's record was missing the name, address and phone number of one emergency contact. Child #4 has been enrolled for over three months.

Each center shall maintain a record for each child enrolled which shall contain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child #2's (age: 4 years) record indicates that the child has twelve food allergies. Child #2's record did not contain a written care plan. Child #2 has been enrolled for two months.
