Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a physical tour of the program .

A complaint inspection was initiated on 8/12/21 and concluded on 9/13/21. The inspector reviewed available information regarding the retention of records, the supervision of children, food safety and ratios pertaining to the complaint investigation. After reviewing the available information part of the complaint is founded.
Information gathered during the inspection determined non-compliance with an applicable standard and a violation were documented on the violation notice issued to the program.

Standard 22VAC40-185-550-M
The center failed to maintain a copy of an injury form, when a child got injured on the playground and needed emergency treatment.
Evidence: Based on interviews with staff and review of the injury reports for the child, the center did not have a copy of the injury report regarding a child falling from a playground structure on 9/11/2020. The center did follow the procedures for a serious injury and the child was transported by EMS to the hospital with a staff member.
Plan of Correction: Karter School has implemented a duplicate copy system for all incident and accident reports to ensure a copy is placed in a child's file and maintained for records. We are moving towards online reporting through google and this should be up and running within the next two weeks.
