Inspector Notes

A complaint investigation concerning behavioral guidance was initiated on 1/13/2023, from a report received by the Fairfax County Licensing office on 1/12/2023. The inspection was conducted and completed on 1/19/2023, in the morning between 10:15 am and 12:00 pm. The inspection included interviews with the management team and teachers. There were 155 children supervised by 28 direct care staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, art, reading, and technology. The preponderance of evidence does not support the allegations and the complaint has been deemed not valid. Non-related violations that were not part of the complaint were cited during the investigation. Areas of non-compliance are identified in the violation notice. Based on observation and interview, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.

Evidence: A bottle of disinfectant spray and all-purpose cleaner was found in the Venice classroom's bathroom, on a shelf above the diaper changing table, not locked.
Plan of Correction: The bottles were immediately removed from the classroom. On 1/19/2023, the bottles were returned to a locked cabinet.

Based on observation and interview, the center failed to ensure that medication was labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.

Evidence: In the Cairo classroom, an inhaler was not labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.
Plan of Correction: The parent did not provide the prescription label/box for the inhaler. The parent was contacted on 1/19/2023 and asked to provide the prescription label/box.
