During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was completed on 2/19/2025, as a part of the licensure period. The on-site inspection began at 10:15 am and ended at 12:45 pm. The inspector reviewed compliance in the areas listed above. There were 156 children present and 32 staff. The inspector reviewed 10 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 02/26/2025. A POC submitted after this date will not appear on the public website.

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of child #5, who had been enrolled for 37 days.
Plan of Correction: Physical was signed by the pediatrician on 2/19/2025.
Standard 8VAC20-780-500-A
Staff shall wash hands with soap and running water before and after each diaper change.

A staff member in a two year old room did not wash hands before or after changing the diapers of 3 consecutive children.
Plan of Correction: Teachers were retrained on 2/19/2025. Teachers reviewed and resigned diaper changing procedure policy.

Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

While changing three children consecutively, a teacher in a two year old classroom, was placing soiled diapers in a pile on the floor.
Plan of Correction: Teachers were retrained on 2/19/2025. Teachers reviewed and resigned diaper changing procedure policy.

Over-the-counter skin products shall not be kept or used beyond the expiration date of the product.

2 diaper ointments were found to be expired in the infant classrooms. One diaper ointment was expired by 5 months, the other diaper ointment was expired by 4 months.
Plan of Correction: Both OTC skin products were replaced with new products on 2/19/2025.
