During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 8/28/25. The on-site inspection began at 11:25am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 10 children present and 3 staff. The inspector reviewed 5 children's records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 9/8/25. A POC submitted after this date will not appear on the public website.

Repeat Violation
The center is required to have employees undergo a background check every five years. The most recent national criminal background check for staff 1 was overdue by a year and 6 months.
Plan of Correction: The center responded with the following: No staff (new or existing) will be permitted to begin or continue employment without a verified documented national background on file.
Standard 8VAC20-780-140-A
Repeat Violation
Physical examinations completed by a physician shall be obtained before the child's attendance or within 30 days after the first day of attendance. There was no physical examination in the record for child 1, who has been enrolled for over one year.
Plan of Correction: The center responded wtih the following: Parents notified immediately. Written request for documents in less than 10 days or child will be unerolled.
Standard 8VAC20-780-245-J-3
Any child for whom emergency medications have been prescribed shall always be in the care of a staff member who has been trained in medication administration. Child 2 is prescribed an emergency medication and was in care on the day of the inspection. There was no staff member trained in the administration of medication present at the center until approximately 11:45am.
Plan of Correction: The center responded with the following: Parents will be notified in writing as well as a "Emergency Plan of Action" change medication policy that aligns with standards.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. On the outside play area, there was a chair with a broken seat, creating a pinching and snagging hazard. There is also a four foot high wooden kitchen playset that is unstable. The playset sways from side to side when touched, creating a toppling hazard.
Plan of Correction: The center responded with the following: Broken chair removed. Kitchen set has been tightened up to be more stable.
