During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 3/24/26. The on-site inspection began at 11:25am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 staff. The inspector reviewed 5 children's records and 4 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards. Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child 2, who had been attending the center for nine months, did not have documentation of an immunization record on file.
Plan of Correction: The center responded with the following: The immunization record for child 2 was obtained from the parent and placed in the child's record.
Standard 8VAC20-780-140-A
Repeat Violation
Physical examinations completed by a physician shall be obtained before the child's attendance or within 30 days after the first day of attendance. There was no physical examination in the record for child 1, who has been enrolled for at least eight months, or for child 2, who has been enrolled for nine months.
Plan of Correction: The center responded with the following: The physicals for child 1 and child 2 were obtained from the parents and placed in each child's record.
Standard 8VAC20-780-550-E
Shelter-in-place procedures shall be practiced a minimum of twice per year. Shelter-in-place procedures were not practiced in 2025.
Plan of Correction: The center responded with the following: Going forward, center management will ensure shelter-in-place drills are conducted at least twice a year and will be documented on the drill log.
Standard 8VAC20-780-60-A
The center shall maintain and keep at the cneter a separate record for each child that contains all the required information. The record for child 1 does not contain the first date of attendance or the addresses for either of the emergency contacts.
Plan of Correction: The center responded with the following: The missing information for child 1 was obtained and documented in the child's record.
Standard 8VAC20-790-600-D-1
All staff who work directly with children shall have current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care. Staff 1's CPR certification was expired by 5 months.
Plan of Correction: The center responded with the following: Staff 1 is scheduled for CPR/FA certifcation in April. Management will ensure that staff 1 is not left alone to care for children until she obtains her recertifcation.
Standard 8VAC20-790-600-D-2
All staff shall have current certification in first aid appropriate to the ages of children in care. Staff 1's first aid certification was expired by 5 months.
