Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.

A renewal inspection was initiated on August 13, 2021 and concluded on August 18, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were three children present, ranging in ages from 11 months to three years, with the provider and an assistant supervising. The point total was 10. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of three children's records were reviewed and one household member's record was reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-111-100-A
Based on record review, the provider failed to ensure that documentation was obtained of a physical examination by or under the direction of a physician prior to the child's attendance or within 30 days after the first day of attendance.

Evidence:
1. There was no physical examination in the record of child #2. Child #2 date of enrollment was 12/7/2020. The provider confirmed there was no physical examination in the child's record.
Plan of Correction: The provider will contact the parent to get documentation of the child's last physical. The provider will ensure all children have a physical prior to attendance or within 30 days after the first day of attendance for any future children enrolled.
Standard 22VAC40-111-90-A
Based on record review, the provider failed to ensure that documentation of immunizations was obtained prior to a child attending.

Evidence:
1. The immunization record for child #1 was dated as printed on the immunization record as 3/9/2021. The child's documented first date of attendance in the record was 2/3/2021.
Plan of Correction: The provider will ensure that documentation of immunizations is obtained prior to a child attending for future enrolled children.
Standard 22VAC40-111-90-C
Based on record review, the provider failed to ensure that documentation of additional immunizations was obtained once every six months for children under the age of two years.

Evidence:
1. The last documentation of immunizations for child #3 was 11/11/2020. Child #2 was enrolled 11/3/2020 and is currently 11 months old. No immunization updates have been obtained by the provider after the 11/11/2020 immunization record.
Plan of Correction: The provider will contact the parent to get a record of updated immunizations.
