Inspector Notes

An unannounced renewal inspection was conducted on 08/21/2023. There were 4 children in care, ages 7 months ? 3 years, totaling 17 points, 1 caregiver and 2 family member children present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 4 children?s records, 2 caregiver?s records, 2 household member?s records and 1 pet record were reviewed. The children were observed during naptime. The inspector arrived for the inspection at 11:15am and departed at 12:29pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on review of two household member records, the provider failed to obtain for each household member a current report of tuberculosis screening form.

Evidence: Household member 2 most recent tuberculosis screening was dated 7/10/2021 and on the day of the inspection no follow up tuberculosis screening was documented.
Plan of Correction: Provider has scheduled follow up tuberculosis screening for household member.
