Inspector Notes
An unannounced monitoring inspection was conducted on 2/22/2024. There were 6 children in care, ages 8 months ? 4 years, totaling 16 points, 2 caregivers and 1 household member present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 3 children?s records, 2 caregiver records and 2 household members records were reviewed. The children were observed during naptime. The inspector arrived for the inspection at 11:50am and departed at 1:00pm.
Discussion on emergency preparedness plan.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

1. Household Member 1 most recent criminal background was dated 1/29/2019 and on the day of the inspection did not have the required 5 year repeat check documented.
2. Household Member 2 most recent criminal background was dated 1/29/2019 and on the day of the inspection did not have the required 5 year repeat check documented.
Evidence: The changing pad in the hallway bathroom had a tear approximately 1/4 inch long on the left side and the padding was exposed.
