Inspector Notes
This monitoring inspection was initiated by licensing staff and concluded on 11/29/2023, between the hours of 10 am to 12 pm. There were 3 children present (10 points), ranging from infant to preschool-age, with 1 caregiver supervising. The children were engaged in the following: morning snack, art, games, freeplay, diaper changing, hand washing, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 4 children records, 1 staff record, and 3 household records were reviewed.
Information gathered during the inspection determined that there were (6) non-compliances with the applicable standards, and code. These violations were documented on the Violation Notice issued to the home.

1. On the date of inspection (11/29/2023), documentation of a physical examination was not available for review for Child #4.
2. The start date for Child #4 was listed as 06/13/2023. Child #4 was in attendance on the date of inspection.

1. The addresses for the two emergency contacts listed for Child #3.
2. The Authorization for Emergency Medical care (found on page 2 of the Child's Record) for Child #2*. This same information was cited during an inspection previously conducted on 05/03/2023.
