Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, and documents review.

A monitoring inspection was conducted on 1/26/2022. The provider was contacted by telephone and a virtual inspection was conducted. There were 4 children present with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 1 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
There was not a documented record of physical examination for Child 2.

Standard 8VAC20-800-90-C
Based on record review, the provider did not obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements.
Evidence:
Child 2's immunization record shave not been update since 4/14/16. Immunizations are required to be updated every six months until a child reached the age of two and once between the child's fourth and sixth birthday.
