Inspector Notes

An unannounced, on-site renewal inspection was conducted on 8/22/25 from 9:45 am to 11:00 am. The inspector reviewed compliance in the areas listed above. There were 48 children present and 8 staff. The inspector reviewed 4 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or
law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
Staff must submit a current Tuberculosis (TB) screening every two years, from the date of the last screening.
An updated TB screening for Staff # 1 was not obtained to this date.
Plan of Correction: Staff # 1's updated TB will be obtained.
Standard 8VAC20-780-40-E
The center did not follow it's own policies and procedures required by the standards, related to medication administration.
The center's medication policy states, "Parents must complete and sign a Medication Authorization with specific times, dosage and dates to be given."
Child # 1's emergency prescription medication was on site without a written parental authorization.
Plan of Correction: We will get the medication form from the parents.
