Inspector Notes

A subsidy health and safety inspection (SHSI) was completed September 6, 2023. Inspector arrived at 11:00am and departed at approximately 1:15pm. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program. The violation is listed on the violation notice issued to the program and was reviewed with the Director at the exit interview.

Based on observation and interview, it was determined that the vendor did not ensure that within the first 90 days of employment all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff.
Evidence:
1. Staff #4 (date of hire 6/14/2019) has not completed the Virginia Preservice Training for Child Care Staff.
2. Staff #1 confirmed that Staff #4 did not complete the Virginia Preservice Training within the first 90 days of employment.

Based on record review and interview, it was determined that the center did not ensure that within 90 days of the date of employment that staff have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Evidence:
1. Staff #3 (date of hire 1//01/2023) did not have a CPR certification within 90 days of the date of employment and was present at the center working with children.
2. Four of the six staff records observed had expired CPR certificates. Staff #4, Staff #5, Staff #6, and Staff #1 had expired certificates dated April 22, 2021. All four staff members were present during the inspection and working directly with children.
3. Staff #1 confirmed Staff #3 did not have a current CPR certification. Staff #1 confirmed that four staff members that were currently working had expired CPR certificates.
