During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on February 4, 2025 and completed on February 4, 2025, as a part of the licensure period. The on-site inspection began at 11:56am and ended at 12:42pm. The inspector reviewed compliance in the areas listed above. There were twelve children present and three caregivers; the point total was thirty-five. The inspector reviewed five children?s records and four caregiver/household member(s) records on-site. This inspection included document review and tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on February 11, 2025. A POC submitted after this date will not appear on the public website.

The program did not have evidence of a testing plan submitted to VDH and/or the program did not have evidence of the implementation of the required testing plan.

Child #1, #2, #4 were sleeping on Fisher Price bouncers and were not provided a crib, cot, rest mat or bed for napping.
Child #2's crib sheet was loosely fitted on the mattress.
Four occupied cribs had three crib mattresses stacked on top of each other in each crib. Child #1, #2, #4 were sleeping on Fisher Price bouncers.
Two V-Tech infant walkers were being used to assist children to learn to walk.
The record for Child #1 was missing the required information for one of two emergency contacts.
Child #1 had two milk bottles stored in a bag with two ice packs under the kitchen cabinet. It had been three hours since Child #1 arrived to the family day home.
