Inspector Notes

An unannounced renewal inspection was conducted on 3/6/2024. At the time of entrance, no children were in care. The record sample size consisted of 5 children's records and the provider's record and the assistant's record. There have been no injuries since the last inspection and no medications are being administered at this time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.

Based on record review and interview it was determined the provider did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. The records for child #2 and child #5, under two years of age, did not contain updated immunization documentation. The documents available were dated 1/31/23 and 3/24/23, respectively.
2. The provider confirmed updated immunization documentation was not available during this inspection,
Plan of Correction: Provider requested updated immunization documentation from parents during this inspection.
