During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced inspection was initiated and completed on 08/07/2025 in response to a complaint received by the licensing office on 07/02/2025 relating to allegations of staff to child ratio. The inspector reviewed 8 children?s records and 4 staff records on-site.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violation is documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on August 15, 2025. A POC submitted after this date will not appear on the public website.

When a group of children receiving care includes children in different age brackets, the age of the youngest child in the group shall be used to determine the staff-to-child ratio that applies to that group.
Upon the arrival of the inspector at 8:50 AM, staff 1 was on duty alone with 8 children, ages 7 months to 3 years.
