During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site code compliance inspection was initiated and completed on 08/07/2025. The on-site inspection began at 8:50 AM and ended at 10:30 AM. The inspector reviewed compliance in the areas listed above. There were 8 children present with 1 staff at the start of the inspection and 8 children with 3 staff at the end of the inspection. The inspector reviewed 8 children?s records and 4 staff records on-site.
This inspection included:

? tour of the facility,
? interviews, and
? observations.

Information gathered during the inspection determined non-compliances with applicable code sections, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on August 15, 2025. A POC submitted after this date will not appear on the public website.

The facility shall have documentation of subsequent background checks conducted every five years.
The most recent completed and signed sworn disclosure statement in the record of staff is dated 2018.

Subsequent TB screenings are required at least every two years from the date of the initial screening, or more frequently if recommended by a physician.
The most recent tuberculosis screenings in the records of staff 1 and 2 are from February 2023.

Bottle fed infants who cannot hold their own bottles shall be held when fed. Bottles shall not be propped or used while the child is in his designated sleeping location.
Upon arrival of the inspector at 8:50 AM, child 8 was in a crib with a bottle.

To label diaper food, bottles at all times.

Prepared infant formula shall be refrigerated, dated, and labeled with the child's name if more than one infant is in care.
There were two infants in care. The prepared bottle for child 8 was not dated and labeled with the child's name.
