Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 12/17/2024. The on-site inspection began at 10:35am and ended at 12pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 5 children present and 2 caregivers; the point total was 15. The inspector reviewed 5 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for Caregiver #1 and Caregiver #2 was not completed and was overdue by five months.

Standard 8VAC20-800-60-B
Children's records are required to contain parental home and employment addresses and phone numbers. The record for Child #2 was missing the home address, phone number and employment information for one parent.
Plan of Correction: The provider will request that parent complete the required information.
Standard 8VAC20-800-770
A working portable battery-operated weather band radio shall be kept in a designated area and be available to caregivers at all times. The battery operated radio was not working during the inspection.
Plan of Correction: The radio could not be opened to replace the batteries. The provider will purchase another battery operated radio.
Standard 8VAC20-800-830-A
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care. There was no documentation of emergency evacuation drills practiced after January 2024.

Documentation of additional immunizations is required once every six months for children under the age of two years. Child #2, 8 months old, last documented immunization was over 6 months ago.
