During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on March 18, 2026. The on-site inspection began at 12pm and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 4 children present and 3 caregivers; the point total was 9. The inspector reviewed 5 children?s records and 3 caregiver/household member(s) records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

caregivers shall have current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care. Two of the three caregivers at the FDH had expired CPR certifications.
Plan of Correction: The provider responded with the following: The two staff members will sign up for CPR class appropriate to the ages of children in care and print the certificate for their file.

All caregivers shall have current certification in first aid appropriate to the ages of children in care. Two of the three caregivers at the FDH had expired first aid certifications.
