Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program.

A renewal inspection was initiated on 8/27/2021 and concluded on 9/1/2021. The Provider was contacted by telephone and a virtual inspection was conducted. There were no children present during the inspection because the Provider is currently in the process of moving. The inspector reviewed compliance in the areas of administration, physical plant, programming, special care and emergencies and nutrition. A total of 2 child records and 1 staff record and 2 household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the provider did not obtain a subsequent tuberculosis (TB) screening every two years for each adult household member.
Evidence: Household member #1's most recent TB screening is dated 3/3/2019.
Plan of Correction: I will have them get it done as soon as possible.
