During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on May 9, 2025. The on-site inspection began at 9:00am and ended at 10:05pm. The inspector reviewed compliance in the areas listed above. There were twelve children present and three caregivers; the point total was 39. The inspector reviewed four children?s records and four caregiver/household member records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

A child's record shall contain documentation of the annual review of the child's emergency contact information.

Child #2 has been in care for one year and seven months and did not have a completed annual review. Child #4 has been in care for one year and three months and did not have a completed annual review.
Plan of Correction: I will have the parents sign at pickup today.
