During the inspection, the inspector reviewed the areas. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was completed on 9/3/2025, as a part of the licensure period. The on-site inspection began at 10:15am and ended at 11:45am. The inspector reviewed compliance in the areas listed above. There were 12 daycare children present, one household child present and three caregivers; the point total was 45. The inspector reviewed five children?s records and three caregiver records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-170-A
Providers must obtain a current tuberculosis screening from a caregiver at the time of hire and prior to coming into contact with children. The tuberculosis screening for Caregiver 2 who has been caring for children since 7/7/25 was dated 7/14/2025.
Plan of Correction: All new caregivers hired will obtain a tuberculosis screening prior to coming in contact with children.
Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for Caregiver #3 was not completed and was overdue by one month.
Plan of Correction: Caregiver will obtain TB screening.
Standard 8VAC20-800-320-B
Child-resistant protective covers larger than 1-1/4 inches in diameter shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age. Two outlet boxes in the nap room that were accessible to children did not have protective covers.
Plan of Correction: The outlet boxes will be removed from the nap room.

Documentation of additional immunizations is required once every six months for children under the age of two years. Child #2, 21 months old, last documented immunization was over 11 months ago.
Plan of Correction: All of the children's immunization records will be reviewed to ensure that they have been updated.
