Inspector Notes

A monitoring inspection was initiated on 4/12/2023 and concluded on 4/12/2023. There were 89 children directly supervised by 17 staff members. Children were observed during snack time, engaged in group activities and outdoor play time. The physical plant, 3 child records, 3 staff records, medications, and evacuation drills were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 22.1-289.035-B-2
Based on a review of staff records and staff interview, staff have not submitted to fingerprinting prior to first day of employment.
Evidence: Staff # 2, date of employment 2/21/2023 does not have results of fingerprinting. The staff was observed caring for children this day.
Plan of Correction: The staff member will be sent home and will not return until the center obtains the fingerprint results. The results will be forwarded to the licensing inspector by email.
Standard 22.1-289.035-B-4
Based on a review of staff records and staff interview, the center has not obtained a copy of a criminal history record information check and a search of the child abuse and neglect registry for staff that have resided in a different state in the past 5 years.
Evidence: Staff #1, date of employment 2/13/2023 stated on the sworn statement that they had resided in the state of California in the past 5 years. No California background checks have been completed.
Staff # 3, date of employment 8/1/2019 stated on the sworn statement that they had resided in the state of Colorado in the past 5 years. The Colorado child abuse and neglect search has not been completed.
Plan of Correction: Management will complete the out of state background checks for the 2 staff members. The results will be filed in the staff's record and forwarded to the licensing inspector.
All staff records will be audited to ensure out of state background checks have been completed by previous administration. (completed by next inspection)
Standard 8VAC20-780-240-B
Based on a review of staff records and staff interview, staff have not completed orientation training no later than seven days of the date of assuming job responsibilities.
Evidence: Staff #1 date of hire 2/13/2023 and staff #2, date of hire 2/21/2023 does not have documentation of orientation training.
Plan of Correction: Management will conduct the orientation with the staff and the orientation document will be filed in each staff's record.
Standard 8VAC20-780-240-E
Based on record review and staff interview, within 30 days of the first day of employment, staff have not completed orientation training in first aid and cardiopulmonary resuscitation (CPR).
Evidence: Staff # , date of hire 2/13/2023 does not have documentation in the completion of orientation in first aid and CPR.
Plan of Correction: Staff will be oriented in first aid and CPR training.

Based on record review, the center has not kept documentation of viewing proof of child's identity and age.
Evidence: Child #1 does not have documentation of proof of identity and age in the record.
Plan of Correction: The parent will be asked to bring in the child's proof of identity to be reviewed.
