Inspector Notes
An unannounced renewal inspection was conducted on December 12, 2023, from 9:45am-12:00pm. At the time of entrance, ( 104 ) children were in care with ( 21 ) staff members present. The inspector reviewed 4 children?s records and 4 staff records. Children were observed during snack time, engaged in group activities, handwriting sentences, cite words, holiday stories and outdoor play time. Medications were observed. Areas of non-compliance are identified on the violation notice. Repeat violation was found and is identified within this report.
The facility is reminded that a complete renewal application must be submitted prior to expiration of the license on January 14, 2024. Renewal applications should be submitted through the online portal available on the public website at childcareva.com.
The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification of the Acknowledgement Form. Once the background check is received by the center, the results must be forwarded to the licensing inspector.

8Based on a review of staff records, the center has not obtained a search of the child abuse and neglect registry for 2 staff that have resided in a different state in the past 5 years.
Evidence: Staff # 1, date of employment 9/20/2021 stated on their sworn disclosure they have resided in the state of Maryland in the past 5 years. No Maryland child abuse and neglect registry has been completed.
Staff # 2, date of employment 7/17/2023 stated on their sworn disclosure they have resided in the state of Maryland in the past 5 years. No Maryland child abuse and neglect registry has been completed.
Evidence: Staff # 1, date of employment 9/20/2021 does not have documentation of a TB screening.
Evidence: staff # 4 TB screening is dated 6/3/2021, there is not an updated TB screening on file within 2 years of the last screening.
Evidence: Staff #2, date of hire 7/17/2023 does not have documentation of completing the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
Evidence: Staff # 1, date of employment 9/20/2021 does not have documentation of completing Child Abuse and Neglect training.
Evidence: Child #5 has a medication authorization for Benadryl from a parent dated 8/16/2023, there is no written medication authorization from the physician for this medication.
Child # 6 has a medication authorization for an Epi Pen from a parent dated 8/9/2023, there is no written medication authorization from the physician for this medication.
Evidence: The dates of the fire drills are maintained on a staff's phone but not documented on the emergency evacuation form with all the required information.
Evidence: Child #5 and #6 have medications on file for a food allergy, there is no written documentation of an allergy action plan from a physician for either child.
