Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and a virtual tour of the program.

A monitoring inspection was initiated on 9/28/2021 and concluded on 9/28/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 7 children present, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 4 staff records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-111-180-A
Based on record review, the provider did not obtain for each adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening.

Evidence:
The records of HH#1 do not include an updated tuberculosis (TB) test/screening. HH#1 has a TB test/screening that expired on 2/11/2021.
Plan of Correction: We will schedule an appointment to get it done.
