Inspector Notes

An unannounced, on-site renewal inspection was conducted on 9/11/2024, as a part of the licensure period. The on-site inspection began at 10:35am and ended at 11:25am. The inspector reviewed compliance in the areas listed above. There were 10 children present and 2 caregivers; the point total was 27. The inspector reviewed 4 children?s records and 2 staff records on-site on 9/11/2024. This inspection included document review policies and procedures, emergency drill logs, authorization forms, general postings and emergency equipment and supplies, a tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the family day home did not obtain documentation of additional immunizations, once every six months for children under the age of two years for a child who is not exempt from the immunization.
Evidence: Child #1 DOB 4/2022, date of last immunizations documented in file 10/2023.
Plan of Correction: Provider will request updated immunizations form parents 9/11/2024.
