Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 11/15/2021 and concluded on 11/30/2021. The director was contacted by telephone to initiate the inspection. There were 3 children present at a total of 8 points and the licensee. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 1 children?s records and 1 staff and one household records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program..

Standard 22VAC40-111-180-A
Based on review of the licensee's and the household's records, the licensee failed to ensure that all repeat TB screenings were obtained.

Evidence:
1. The licensee's most recent TB screening was completed on 11/21/2018, the requirement is for the TB to be repeated every two years.
2. The household Member # 1 most recent TB screening was completed on 2/15/2019
Plan of Correction: The licensee will obtain a new TB along with the household member. The time frames will be monitored.
