Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on April 9, 2021 and concluded on April 12, 2021. The provider was contacted by telephone to initiate the inspection. There were 5 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 household member records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

The record for child #1 does not contain a physical examination by a physician before the first day of attendance.

The record for household member #3, who turned 18, does not contain documentation of an initial TB screening.
Evidence:
The records for household members #1 and #2 do not have documentation of a current report of TB screening. The reports in their records are expired.
Evidence:
There is no record of emergency evacuation drills being practiced monthly. Caregiver #1 confirmed they are not being conducted.

There is no record of shelter in place being practiced twice a year. Caregiver #1 confirmed they are not being conducted.
