Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on April 9, 2021 and concluded on April 12, 2021. The provider was contacted by telephone to initiate the inspection. There were 5 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 household member records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard 22VAC40-111-100-B
Based on review of three children's records, one record does not contain the required documentation before first day of attendance.
Evidence:
The record for child #1 does not contain a physical examination by a physician before the first day of attendance.
Plan of Correction: I will get a copy of their physical and put it in their record.
Standard 22VAC40-111-170-A
Based on review of three household member records, one does not have contain the required documentation for an initial tuberculosis (TB) screening.
Evidence:
The record for household member #3, who turned 18, does not contain documentation of an initial TB screening.
Plan of Correction: I will get a TB screening for her and put the results in her record.
Standard 22VAC40-111-180-A
Based on review of three household member records, two do not contain the required documentation for subsequent tuberculosis (TB) screenings.
Evidence:
The records for household members #1 and #2 do not have documentation of a current report of TB screening. The reports in their records are expired.
Plan of Correction: We will have another TB screening completed.
Standard 22VAC40-111-830-A
Based on review of emergency procedures, emergency evacuation procedures are not being practiced monthly.
Evidence:
There is no record of emergency evacuation drills being practiced monthly. Caregiver #1 confirmed they are not being conducted.
Plan of Correction: We will begin immediately to practice monthly fire drills and record them.

Based on review of emergency procedures, emergency procedures are not being practiced monthly.
Evidence:
There is no record of shelter in place being practiced twice a year. Caregiver #1 confirmed they are not being conducted.
Plan of Correction: We will conduct shelter in place drills twice a year and record them.
Standard 22VAC40-191-40-D-4-a
Based on review of three household member records, one does not contain the required background checks.
Evidence:
The record for household member #3 who turned 18 does not have documentation of a signed sworn statement or letter of eligible fingerprint.
Plan of Correction: I will make sure the background checks are completed and send the results to my inspector when received.
