Inspector Notes
An unannounced, on-site focused inspection was completed on July 18, 2024 as a part of the provisional licensure period. The on-site inspection began at 10:00am and ended at 10:45am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers. The inspector reviewed 5 children?s records, 2 caregiver records and 1 household member record. This inspection included document review, and tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

HHM#1 did not sign a new Sworn Statement or affirmation every five years. The current signed Sworn Statement in their record expired 5/18/2024.
Based on review of five children's, not all records were complete.
Evidence
The record for child #3 did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.
Based on review of 5 children's records, the provider did not annually review the child's record with the parent.

The last parental review of child #1's record was documented as 8/3/2022.

Child #2, under the age of 2, first day of attendance 12/6/2023, last immunization update 4/10/2023.
Based on observation, snack were provided by the family day home and the menu for the current week period did not list substituted food.
Evidence
There were two menus posted that each listed different items being served to the children. Snack being served during the inspection was strawberries, which was not listed on either posted menu.
