Inspector Notes

An unannounced, on-site focused inspection was completed on July 18, 2024 as a part of the provisional licensure period. The on-site inspection began at 10:00am and ended at 10:45am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers. The inspector reviewed 5 children?s records, 2 caregiver records and 1 household member record. This inspection included document review, and tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.036-A
Based on review of household member (HHM), all HHM did not complete a Sworn Statement or affirmation every five years.
Evidence
HHM#1 did not sign a new Sworn Statement or affirmation every five years. The current signed Sworn Statement in their record expired 5/18/2024.
Plan of Correction: My household member will sign the sworn statement and I will put it in his record.
Standard 8VAC20-800-60-B
REPEAT VIOLATION
Based on review of five children's, not all records were complete.
Evidence
The record for child #3 did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.
Plan of Correction: I will review all the records to ensure they contain the required information.
Standard 8VAC20-800-780-B
REPEAT VIOLATION
Based on review of 5 children's records, the provider did not annually review the child's record with the parent.
Evidence
The last parental review of child #1's record was documented as 8/3/2022.
Plan of Correction: I will get the required information and make sure to keep the records updated.
Standard 8VAC20-800-90-C
Based on review of 5 children's records, the family day home did not obtain documentation of additional immunizations for a child under the age of two, every six months.
Evidence
Child #2, under the age of 2, first day of attendance 12/6/2023, last immunization update 4/10/2023.
Plan of Correction: I will create a system to make sure to keep track of when updated immunizations are due.
Standard 8VAC20-800-920
REPEAT VIOLATION
Based on observation, snack were provided by the family day home and the menu for the current week period did not list substituted food.
Evidence
There were two menus posted that each listed different items being served to the children. Snack being served during the inspection was strawberries, which was not listed on either posted menu.
Plan of Correction: I will only post one menu and make sure it reflects any food we substitute for that day and will make sure the families are aware of any changes.
