Inspector Notes

A subsidy health and safety (SHSI) inspection was initiated and concluded on 6/13/2023. There were 8 child present with 2 caregivers supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Based on a review of three (3) caregiver records and interview, the vendor did not ensure that one (1) caregiver who works directly with children annually completed the department's health and safety update course as required.

Evidence:
1. The record of caregiver #3 contained an annual health and safety course certificate dated 8/31/2021.
2. The vendor acknowledged that the record did not contain a 2022 course certificate.
