Inspector Notes
An unannounced monitoring inspection was conducted on 5/29/2024. The licensing inspector was on site from approximately 10:09am until 11:40am. There were three children present with one caregiver supervising, totaling 11 points. Children were observed in variety of activities including playtime, learning numbers and letters, indoor play with building blocks. Caregiver interacted with children in a positive manner. Inspector interacted with caregiver and children in care.
The inspector reviewed compliance in the areas of administration, personnel, household members, caregiver training, physical equipment and environment, emergencies, nutrition, medication administration and care of children. A total of three child records, one caregiver record and one household member record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions, please contact the licensing inspector.

1. Caregiver #1 had a criminal history background check which was last conducted on 05/13/2019
2. Caregiver #1 had a central registry background check which was last conducted on 04/12/2019
3. Provider acknowledged the background checks have not been renewed.
2.CRS HAS BEEN DONE .PLAN OF CORRECTION I WILL REVIEW FILES MORE OFTEN SO THIS ISSUE WILL NOT HAPPEN AGAIN.
Evidence:
1. A diaper cream was observed at the home that was labeled and used for child #1. Upon record review, there was no parent authorization for the diaper cream.
2. The provider acknowledged that they were using a diaper cream for the child without written parent authorization to apply the diaper cream.
