During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated on December 6, 2024 and completed on December 6, 2024, as a part of the provisional licensure period. The on-site inspection began at 10:25am and ended at 11:10am. The inspector reviewed compliance in the areas listed above. There were two children present and one caregiver; the point total was seven. The inspector reviewed two children?s records on-site. This inspection included document review and a tour of the family day home.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

There was no physical exam record available for Child A, who had been in care for 74 days.
Plan of Correction: Will ask parent to provide the physical examination.
Standard 8VAC20-800-60-B
REPEAT VIOLATION:

Children's records are required to contain the name, office address and telephone number of the child's physician and the name and address for an emergency contact.

The record for Child B was missing the name, office address and telephone number of the child's physician. Child B was also missing the complete name and address for the child's second emergency contact. The child was present during the inspection and had been in care for 74 days.
Plan of Correction: Will have the parent complete the necessary information.
