Inspector Notes

A Subsidy Health and Safety inspection was conducted on 3/20/2024 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Amended to remove previously cited violation.

Based on attendance record review, the provider/vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care.
Evidence: Attendance record lists children's name daily and not date/time of arrival and departure.
Plan of Correction: Provider will change attendance record form to include daily time of arrival and departure for each child in care as it occurs.
